The Super Dentists realize you have better things to do than fill out paperwork during your first visit, so you can fill out your required paperwork at your convenience! Complete your forms in advance of your first visit, and you’ll be ready to go. Once you’ve become a regular patient, we encourage you to refer a friend or family member to The Super Dentists!
