Is a purchase required to win?
No. If you do not want to purchase The Collector’s Edition, you will receive a magic augmented reality sticker at no cost. *See official rules for details.
How will I know I’ve won? What does the golden sticker look like?
Each toothbrush comes with a magical augmented reality sticker featuring The Super Dentists’ character, The Tooth Keri. Usually, she wears a white hat with a yellow band and a RED medallion in the center. In the winning toothbrush packages, the sticker, located behind the toothbrush in one of the packages has been replaced with a sticker of the Tooth Keri wearing a white hat with a yellow band and a GOLD medallion (see photo below right). There is also a slip of paper with a number inside that lets the winner know they’ve won (see photo below) and instructions to contact our office.
What are the prizes?
If you find a golden sticker, you will win 4 premium annual passes (2 adults + 2 children) to one of these five Southern California attractions: Disneyland, Universal Studios Hollywood, Legoland California, SeaWorld San Diego or The World Famous San Diego Zoo! (Winners are not able to change the number of adult and children’s passes nor are they able to select a different prize.)
Each of the five winners will receive a special behind-the-scenes tour of The Super Dentists operations and a lifetime supply of The Super Toothbrush. (4 toothbrushes per year for as long as they are an ACTIVE Super Dentists patient – meaning they keep their regularly scheduled semi-annually appointments).
When will I receive my prize?
Once all golden sticker winners have been identified, The Super Dentists will award prizes in random order during a live drawing in early 2020. Exact date TBD.
What if I don’t have four people in my family? –
If a winning family consists of less than four people, The Super Dentists reserves the right to award the exact number of passes of adults/children in a family and is not required to award additional passes.
What if I have more than 4 people in my family?
No additional passes will be awarded.
Are there any eligibility requirements?
The winner must be at least 18 years old and be a resident of Southern California, live in ZIP codes 90000 to 93599, and be able to provide proof of Southern California residency. A child can find the sticker, but their parent or guardian who has legal custody of the child, and is 18 or over, must contact The Super Dentists to let us know they won and comply with the verification process and required paperwork. A family can only win one prize.
How will you verify that I’ve won?
When a winner contacts The Super Dentists office to let us know they won, they will provide their full name, address, phone number and email address, along with a photo or video (cell phone photo is ok) of the Golden ToothKeri sticker with the Super Dentists ToothKeri character wearing a white hat with a yellow band and a gold medallion and a separate photo of the notification that says Congratulations Golden Sticker Winner and contains a number at the bottom of the paper. If a parent videos a child opening the package and it is a winner, that can also partly serve as a method of verification. The Super Dentists will require the winner to bring in the original sticker and original congratulations message with the number to The Super Dentists Command Center, 9737 Aero Drive, Suite 100, San Diego, CA 92123, to verify that it is, in fact, a winner. Please note you must present the physical golden sticker to The Super Dentists to receive your prize. No copies or substitutions permitted. Subject to full verification, prizes will be awarded in 2020.
What if I have an issue?
Once you receive the annual passes, you are responsible for registering them, creating an account, making reservations and finding out the answers to any questions related to visiting the park.
The Super Dentists is not able to contact any of the theme parks on the winning family’s behalf to ask questions, serve as a concierge, make reservations, report lost or stolen passports, make special accommodations or resolve any issues that arise in relation to getting to or from the theme park or visiting the theme park, etc. Once The Super Dentists conveys the passes, the winner(s) assume all responsibility from that date on.
What is not included in the prize?
Not responsible for any incidental costs and expenses, including time off from work, including, but not limited to meals, snacks, hotel rooms, parking costs, travel costs, transportation (gas, tolls, and any other transportation expenses) to and from the attraction spending money, tickets or passes for additional guests, the cost of any upgraded or additional special experiences, character experiences, special events, fastpass access, guest services rentals, childcare, fees for lost or stolen passports, spending money, souvenirs, merchandise, and/or any other costs outside of the cost of the four annual passes.
What am I responsible for?

The 4 annual passes will be conveyed in the form of gift cards for the attraction the winner won OR the 4 annual passes will be purchased in the names of the winning family. The Super Dentists reserves the exclusive right to decide how the prize will be awarded once all winners are identified. All decisions by The Super Dentists are final.
The winners will be required to come to The Super Dentists Command Center, 9737 Aero Drive, Suite 100, San Diego, CA 92123, to bring in their Golden Sticker, to sign proof of receipt, other required paperwork as noted below and to take a photo.
Once the winners have received their passes and signed the documents The Super Dentists will not be responsible for replacing passes that are lost or stolen or replace them for any other reason.
What will I have to fill out?
Winners will be asked to bring in the original Golden ToothKeri sticker AND the original notification with a unique serial number that says Congratulations! Golden Sticker Winner.
The winner will be required to complete:
- An affidavit of eligibility to be notarized by the winner at their own cost
- W-9 for tax purposes
- Non-revocable Media/photography release allowing the use of their name(s), age, city, state, and photo.
(All parents and legal guardians of minors must sign on behalf of any minor.)
- Acknowledgment that winners will not transfer or sell their passes
Additional forms may be required and a photo of the winning family will be taken at that time to be shared in The Super Dentists e-newsletter, on social media, to be sent out to media, in marketing materials, and in any other uses as determined by The Super Dentists.
Can I transfer or sell my passes? Can I give my pass to someone else?
The passes and their benefits are non-transferable, subject to restrictions and change without notice, and may not be combined with any other discount, offer or promotion. The passes are for personal use only, and the passes and their benefits may not be used for commercial purposes or for the benefit of others, including, without limitation, the purchase of merchandise with the intent to resell such merchandise or the sharing of benefits with persons who are not the Passholder. Any violations of these provisions renders the passes void, and The Super Dentists reserves the right to cancel all unused passes.
How long will the contest run?
The Super Dentists Golden Sticker Contest will run from November 18, 2019 through early 2020. Please check The Super Dentists website and social media for contest updates.
When will the prizes be awarded?
The prizes will be awarded within 90 days of the contest ending. The Super Dentists reserves the right to award prizes earlier than 90 days or to extend the prize delivery date if deemed necessary, as long as The Super Dentists notifies winners in writing (via email to the email address provided by the winners).
What happens if all the stickers are not found?
If all winners have not been identified by the end of the contest, The Super Dentists reserves the right to award or not award that prize at their sole discretion. The decision will be communicated via our website at TheSuperDentists.com/TheSuperToothbrush.
How many golden stickers are there? – There are five golden stickers in all.
How will I know if all the stickers have been found?

NO PURCHASE NECESSARY TO ENTER OR WIN. THIS SWEEPSTAKES IS OPEN TO LEGAL RESIDENTS OF SOUTHERN CALIFORNIA TERRITORY DEFINED AS UNITED STATES POSTAL ZIP CODES OF 90000 TO 93599 (THE SOCAL TERRITORY) WHO ARE AT LEAST 18 YEARS OF AGE OR OLDER ON THE DATE OF ENTRY.

1) SWEEPSTAKES PERIOD: The Super Dentists Golden Sticker Sweepstakes (the “Sweepstakes” or the “Promotion”) starts November 18, 2019 at 12:01 A.M. PST and ends in early 2020 at a date determined by The Super Dentists (the “Sweepstakes Period”). All prizes must be claimed on or before April 30, 2020 by 5:00 P.M. PST (the “Prize Claim Period”) with no exceptions or waiver of these requirements.
2) ELIGIBILITY: TO BE ELIGIBLE TO WIN, YOU MUST BE A LEGAL RESIDENT OF THE SOCAL TERRITORY AND BE AT LEAST 18 YEARS OF AGE OR OLDER ON THE DATE OF ENTRY (“Entrant”). The officers, directors and employees of the Super Dentists (the “Sponsor”) its subsidiaries and affiliated organizations, advertising and promotion agencies, persons engaged in the development, production or distribution of materials for this promotion (collectively, the “Promotion Parties”) and the immediate family members (parent, child, sibling, and spouse of each) of and/or persons living in the same household as such individuals (whether related or not) are not eligible to participate. This Sweepstakes is void where prohibited by law. All potential winners and participants are subject to eligibility verification and compliance with these Official Rules.
3) HOW TO PARTICIPATE: NO PURCHASE IS NECESSARY TO PARTICIPATE OR WIN. To participate, you can collect or obtain a Super Dentists Tooth Keri Augmented Reality Sticker (the “Sweepstakes Sticker”) upon request and available at any of The Super Dentists six offices in the packages of The Super Toothbrush Collector’s Edition. No Promotional Packages are available online or offered for distribution through any retailer or wholesale outlets other than Packages available for sale at any of the Super Dentists six offices: Carmel Valley, Chula Vista, Eastlake, Kearny Mesa, Escondido or Oceanside, located in San Diego, CA. The ToothKeri Augmented Reality Sticker can be obtained over the counter at any one of the Super Dentists offices without the purchase of any toothbrush products (“Over The Counter Handouts”). There are no limits on the number of stickers that may be collected overall, however, the Super Dentist reserves the right to limit distribution of Over The Counter Handouts to two per adult person per visit to any one of the Super Dentist offices in any twenty four hour period. No ToothKeri Augmented Reality Stickers will be mailed or issued through any other channels. ALL TOOTHKERI AUGMENTED REALITY STICKERS WILL BE DISRIBUTED WHILE SUPPLIES LAST OR THROUGH THE END OF THE SWEEPSTAKES PERIOD.
4) DETERMINATION OF WINNERS: THERE ARE A TOTAL OF FIVE WINNING STICKERS THAT HAVE BEEN RANDOMLY DISTRIBUTED IN ONE OF THE FOUR TOOTHBRUSH PACKAGES INSIDE THE PROMOTIONAL PACKAGES AND THROUGH OVER-THE-COUNTER HANDOUTS. ALL STICKERS WILL BE MADE AVAILABLE AT THE SAME TIME AT THE START OF THE SWEEPSTAKES PERIOD. However, it is possible that a Promotional Package or an Over-The Counter Handout may not be claimed by the end of the Sweepstakes Period. Sweepstakes Stickers obtained after the close of the Sweepstakes Period are deemed null and void. Only the Sweepstakes Sticker featuring the Super Dentist character The Tooth Keri wearing a white hat with a yellow band and featuring a gold medallion replacing the traditional red medallion on the character are designated a winner (“Golden Sticker”). For security purposes, each Golden Sticker is accompanied by an additional notification with a unique serial number that identifies the winning prize. Purchase of The Super Toothbrush Collector’s Edition product will not increase or enhance the odds of winning a prize.
4) ALL PRIZES AWARDED ARE BASED ON THE ASSIGNED UNIQUE SERIAL NUMBER ASSOCIATED WITH THE PRIZE AWARD: THERE WILL BE FIVE WINNING STICKERS RANDOMLY DISTRIBUTED. Sweepstake Prizes shall be distributed based on the winning Golden Stickers coming forth to claim a prize before the end of the Prize Claim Period and that matches the serial number on file for the specified prize. All claims for a prize require the in-person presentation of the original winning Golden Sticker and accompanying original unique serial number at the The Super Dentists Command Center, located at 9737 Aero Dr #100, San Diego, CA 92123 by 5:00 P.M., January 31, 2020. At the time of presentation, the party claiming to be the winner is responsible for obtaining a written receipt notating the time of delivery of the winning Golden Sticker, their full legal name and contact information including a valid email address, and telephone number for further contact. The following is the scheduled allocation of prizes and awards. For security purposes the list of serial numbers shall be randomly assigned to each of one of the five prizes. The assignment list shall then be placed in a secured and confidential location until the close of the Sweepstakes Period. The list of winning serial numbers associated with each prize shall be released to the public within three days after the close of the Sweepstakes Period. The list shall be posted on the Sponsor’s Facebook page and the Sponsor’s website:

5) UNCLAIMED PRIZES; SECOND CHANCE DRAWING. All prizes not claimed by the Prize Claim Period are deemed forfeited by any alleged winner (“Forfeited Prizes”). Upon the close of the Prize Claim Period, all Forfeited Prizes shall be awarded via a random drawing held live on Facebook (“Second Chance Drawing”). Any person desiring to enter the Second Chance Drawing can go to https://SuperDentists.com/GoldenStickerSweepstakes and follow the on-screen instructions. You must complete and submit the Super Dentists Second Chance Drawing Entry Form (the “Second Chance Entry”) as instructed to be eligible to enter the drawing for any remaining and unclaimed prizes.
ALL PERSONS WHO HAVE SUBMITTED A CLAIM AS A PROSPECTIVE WINNER CLAIMING TO HOLD A GOLDEN STICKER ARE INELIGIBLE TO WIN MORE THAN ONE SWEEPSTAKES PRIZE OR ENTER THE SECOND CHANCE DRAWING.
ANY PRIZE NOT DISTRIBUTED THROUGH THE GOLDEN STICKER SWEEPSTAKES AND THE SECOND CHANCE DRAWING MAY BE DONATED TO CHARITY AT THE SOLE DISCRETION OF THE SPONSOR.
6) DISCLOSURES; ODDS OF WINNING. Many will enter and few will win. The odds of winning any prize are approximately 1: 504. There are no means of estimating the odds of winning in any Second Chance Drawing, or whether there will be the need for a second chance drawing. The odds of winning a prize depend on the total number of eligible entries received.
7) AGREEMENT OF COMPLIANCE WITH OFFICIAL RULES: By submitting any entry to this Sweepstakes, Entrant agrees to comply with these Official Rules. Entries that do not comply with these Official Rules and Guidelines and Content Restrictions above, as determined in Sponsor’s sole discretion, will be disqualified from the Sweepstakes. Decisions of the Sponsor are final and binding. By entering this Promotion, each Entrant agrees to grant the Sponsor, subsidiaries, and affiliates and its agents and designees, the exclusive, worldwide, perpetual, irrevocable, royalty-free, sub-licensable right to exercise the copyright, publicity, database rights and any other rights Entrant has concerning the use of their name and likeness as a participant in the Sweepstakes.
8) WINNER NOTIFICATIONS, DOCUMENTATION, CONFIRMATION, AND PRIZE FULFILLMENT: Any potential Golden Sticker Winner will be notified within three (3) business days of their submission of a Golden Sticker of verification of the authenticity of the submitted Golden Sticker and verification of Entrant eligibility. Winners of prizes with a fair market value of $600 or greater must execute and return an IRS Form W-9 and an affidavit of eligibility and liability release, as well as any additionally required forms, and must provide a photocopy of a legal document showing Southern California residence in the Zip Code of 90000 to 93599 and a photocopy of a legal document showing the winner’s social security number within seven (7) calendar days after receiving such forms or the prize will be forfeited and an alternate winner will be randomly selected from the Second Chance Drawing entries. Winners agree to accept the estimate of fair market value of the prize. Winners of prizes with a fair market value of less than $600 may be requested to execute and return an affidavit of eligibility (at their own expense) and liability release and provide a photocopy of a legal document showing Southern California residence in Zip Code 90000 to 93599. We may require proof of each winner’s own social security number before a prize can be released to the winner.
9) ALL PRIZES MUST BE ACCEPTED AS AWARDED: Prizes are not transferable, redeemable for cash or exchangeable for any other prize. Winners must claim the prize within the Prize Claim Window without exception. If a winner fails to redeem the prize within the Prize Claim Period, the winner forfeits the prize and no other prize will be awarded in its place. We retain the right to substitute a prize with another similar prize of equal or greater value in the event the original prize offered is not available due to circumstances beyond our control.
10) DELIVERY OF PRIZES: All prizes will be picked up at The Super Dentists Command Center located at 9737 Aero Dr #100, San Diego, CA 92123 by 5:00 P.M, April 30, 2020, unless otherwise noted. We are not responsible for any lost, stolen, misdirected or damaged mail. Prizes lost or damaged after delivery to the Entrant will not be replaced.
11) THE SPONSOR HAS NO RESPONSIBILITY FOR PAYMENT OF TAXES AND RELATED EXPENSES: Each winner is solely responsible for reporting and paying any and all applicable taxes related to the prize(s) and paying any expenses associated with any prize which are not expressly included as part of the prize.
12) RELEASE OF LIABILITY: All winners and their guests, by acceptance of any prize, agree to release and forever hold harmless the Sponsor and their affiliated prize partners, their affiliated companies, their agents, officers, employees, affiliates and assigns for any injuries, accidents, misfortunes, or causes of action arising from or in connection with participation in any contest, acceptance of any prize, use of any prize, or while traveling to or from any prize-related activity. The Sponsor has the sole discretion to terminate the Sweepstakes at any given time or extend the dates of the Sweepstakes. The Sponsor retains the right to amend these rules at any time for any reason. The Sponsor reserves the right to substitute prizes of equal or greater value upon notice or change to the sweepstakes rules. Any ties, disputes, conflicts, questions or concerns regarding our sweepstakes and/or their associated prizes will be handled by the Director of Marketing of the Sponsor whose decisions are final and indisputable.
13) GENERAL: Except as determined by the Sponsor in its sole discretion, no substitution of prize or award shall be offered and no transfer of prize or award to a third party is permitted. All federal, state, and other tax liabilities arising from this Sweepstakes are the sole responsibility of prize winners. No more than the listed number of prizes and awards will be provided.
14) CONDUCT: The Sponsor reserves the right, at its sole discretion, to disqualify any individual: (a) suspected of tampering with the entry process or the operation of the Sweepstakes; (b) to be acting in any manner deemed by the Sponsor to be in violation of the Official Rules; (c) to be acting in any manner deemed by the Sponsor to constitute unsportsmanlike or disruptive behavior, or with intent to annoy, abuse, threaten or harass any other person. CAUTION: ANY ATTEMPT BY A USER OR ENTRANT OR ANY OTHER INDIVIDUAL TO DELIBERATELY DAMAGE, COPY, OR ALTER ANY SWEEPSTAKES TICKER OR UNDERMINE THE LEGITIMATE OPERATION OF THIS SWEEPSTAKES IS A VIOLATION OF CRIMINAL AND CIVIL LAWS AND SHOULD SUCH AN ATTEMPT BE MADE, THE SPONSOR RESERVES THE RIGHT TO SEEK DAMAGES AND COSTS (INCLUDING WITHOUT LIMITATION, ATTORNEYS’ FEES) FROM ANY SUCH PERSON TO THE FULLEST EXTENT PERMITTED BY LAW.
15) LIMITATIONS OF LIABILITY: The Promotion Parties assume no responsibility or liability for (a) any incorrect or inaccurate entry information, or for any faulty, failed, garbled or jumbled electronic data transmissions; (b) any unauthorized access to, or theft, destruction or alteration of entries at any point in the operation of this Sweepstakes; (c) any technical malfunction, failure, error, omission, interruption, deletion, defect, delay in operation or communications line failure, regardless of cause, with regard to any equipment, systems, networks, lines, satellites, servers, computers or providers utilized in any aspect of the operation of the Sweepstakes; (d) inaccessibility or unavailability of the Internet or any combination thereof; or (e) any injury or damage to the participant’s or to any other person’s computer which may be related to or resulting from any attempt to participate in the Sweepstakes or to download any materials from the Sweepstakes. If, for any reason, the Sweepstakes is not able to run through completion for reasons which may include, without limitation, infection by computer virus, bugs, tampering, unauthorized intervention, fraud, technical failures, or any other causes which may corrupt or affect the administration, security, fairness, integrity or proper conduct of this Sweepstakes, then the Sponsor reserves the right, in its sole discretion, to cancel, terminate, modify or suspend the Sweepstakes in whole or in part, without further liability to the entrants or third parties. 16) DISPUTES: The Sweepstakes is governed by the laws of the State of California. All federal, state and local laws and regulations apply. The construction, validity and interpretation of the Promotion shall be governed by the laws of the California, without giving effect to any choice of law or conflict of law rules. All disputes, claims and causes of action arising out of or related to the Sweepstakes shall be resolved in the applicable court within the County of San Diego, California.
